In preparation for expanded use in the kernel, make getname_kernel()
more useful by allowing it to handle any legal filename length.

Thanks to Guenter Roeck for his suggestion to substitute memcpy() for
strlcpy().

There are several areas in the kernel that create temporary filename
objects using the following pattern:

	int func(const char *name)
	{
		struct filename *file = { .name = name };
		...
		return 0;
	}

... which for the most part works okay, but it causes havoc within the
audit subsystem as the filename object does not persist beyond the
lifetime of the function.  This patch converts all of these temporary
filename objects into proper filename objects using getname_kernel()
and putname() which ensure that the filename object persists until the
audit subsystem is finished with it.

Also, a special thanks to Al Viro, Guenter Roeck, and Sabrina Dubroca
for helping resolve a difficult kernel panic on boot related to a
use-after-free problem in kern_path_create(); the thread can be seen
at the link below:

a) make it accept ERR_PTR() as filename (and return its PTR_ERR() in that case)

Enable recording of filenames in getname_kernel() and remove the
kludgy workaround in __audit_inode() now that we have proper filename
logging for kernel users.

In all likelihood there were some subtle, and perhaps not so subtle,
bugs with filename matching in audit_inode() and audit_inode_child()
for some time, however, recent changes to the audit filename code have
definitely broken the filename matching code.  The breakage could
result in duplicate filenames in the audit log and other odd audit
record entries.  This patch fixes the filename matching code and
restores some sanity to the filename audit records.

In order to ensure that filenames are not released before the audit
subsystem is done with the strings there are a number of hacks built
into the fs and audit subsystems around getname() and putname().  To
say these hacks are "ugly" would be kind.

This patch removes the filename hackery in favor of a more
conventional reference count based approach.  The diffstat below tells
most of the story; lots of audit/fs specific code is replaced with a
traditional reference count based approach that is easily understood,
even by those not familiar with the audit and/or fs subsystems.

What we want is to have non-counting references to children in
pagecache of parent directory, and avoid picking them after a child
has been freed.  Fine, so let's just have ->d_prune() clear
parent's inode "has directory contents in page cache" flag.
That way we don't need ->d_fsdata for storing offsets, so we can
use it as a quick and dirty "is it referenced from page cache"
flag.

it's not only badly racy, it's actually dead code - the call in
ll_invalidate_aliases() is unreachable.  For an alias of our inode
to be root dentry, the inode would have to be its ->d_inode, aka
the root inode, and we never call ll_invalidate_aliases() for that.

Replace the old ns->bacct only with NULL and only if it still points
to acct.  And assign the new value to it *before* calling acct_kill()
in acct_on().  That way we don't need to pass the new acct to acct_kill().

The value resulting from the SECCOMP_RET_DATA mask could exceed MAX_ERRNO
when setting errno during a SECCOMP_RET_ERRNO filter action.  This makes
sure we have a reliable value being set, so that an invalid errno will not
be ignored by userspace.

  md: When RAID5 is dirty, force reconstruct-write instead of read-modify-write.

Causes an RCW cycle to be forced even when the array is degraded.
A degraded array cannot support RCW as that requires reading all data
blocks, and one may be missing.

Forcing an RCW when it is not possible causes a live-lock and the code
spins, repeatedly deciding to do something that cannot succeed.

So change the condition to only force RCW on non-degraded arrays.

Dave Chinner has properly pointed out that this is a deadlock in the
reclaim code because ext4 doesn't submit pages which are marked by
PG_writeback right away.

The heuristic was introduced by commit e62e384 ("memcg: prevent OOM
with too many dirty pages") and it was applied only when may_enter_fs
was specified.  The code has been changed by c3b94f4 ("memcg:
further prevent OOM with too many dirty pages") which has removed the
__GFP_FS restriction with a reasoning that we do not get into the fs
code.  But this is not sufficient apparently because the fs doesn't
necessarily submit pages marked PG_writeback for IO right away.

ext4_bio_write_page calls io_submit_add_bh but that doesn't necessarily
submit the bio.  Instead it tries to map more pages into the bio and
mpage_map_one_extent might trigger memcg charge which might end up
waiting on a page which is marked PG_writeback but hasn't been submitted
yet so we would end up waiting for something that never finishes.

Fix this issue by replacing __GFP_IO by may_enter_fs check (for case 2)
before we go to wait on the writeback.  The page fault path, which is
the only path that triggers memcg oom killer since 3.12, shouldn't
require GFP_NOFS and so we shouldn't reintroduce the premature OOM
killer issue which was originally addressed by the heuristic.

As per David Chinner the xfs is doing similar thing since 2.6.15 already
so ext4 is not the only affected filesystem.  Moreover he notes:

: For example: IO completion might require unwritten extent conversion
: which executes filesystem transactions and GFP_NOFS allocations. The
: writeback flag on the pages can not be cleared until unwritten
: extent conversion completes. Hence memory reclaim cannot wait on
: page writeback to complete in GFP_NOFS context because it is not
: safe to do so, memcg reclaim or otherwise.

When locking a GPIO line as IRQ, we go to lengths to
double-check that the line is really set as input before
marking it as used for IRQ. This is not good on GPIO chips
that can sleep, because this function is called in IRQ-safe
context. Just skip this if it can't be checked quickly.

Currently this happens on sleeping expanders such as STMPE
or TC3589x:

qxl_release_map will enter an atomic context, but since we still need to
alloc memory for BOs, we better delay that until we have everything we
need, in case we need to sleep inside the allocation.  This avoids the
Sleep in atomic state below, which was reported by Mike.

pppol2tp_release uses call_rcu to put the final ref on its socket. But
the session object doesn't hold a ref on the session socket so may be
freed while the pppol2tp_put_sk RCU callback is scheduled. Fix this by
having the session hold a ref on its socket until the session is
destroyed. It is this ref that is dropped via call_rcu.

Sessions are also deleted via l2tp_tunnel_closeall. This must now also put
the final ref via call_rcu. So move the call_rcu call site into
pppol2tp_session_close so that this happens in both destroy paths. A
common destroy path should really be implemented, perhaps with
l2tp_tunnel_closeall calling l2tp_session_delete like pppol2tp_release
does, but this will be looked at later.

syzbot caught an infinite recursion in nsh_gso_segment().

Problem here is that we need to make sure the NSH header is of

allocate_data_block will submit all sequential pending IOs sorted by a
FIFO list, If we failed to submit other user's IO due to unaligned write,
we will retry to allocate new block address for current IO, then it will
initialize fio.list again, if fio was in the list before, it can break
FIFO list, result in above panic.

This is because in recovery flow of __exchange_data_block, we didn't pass olen to
__roll_back_blkaddrs, instead we passed len, which indicates wrong array size, result
in copying random block address into dnode page.

Later, once that random block address was accessed by is_checkpointed_data, it can
cause NULL pointer dereference.

Increase kasan instrumented kernel stack size from 32k to 64k. Other
architectures seems to get away with just doubling kernel stack size under
kasan, but on s390 this appears to be not enough due to bigger frame size.
The particular pain point is kasan inlined checks (CONFIG_KASAN_INLINE

Connecting a vCPU to a XIVE KVM device means establishing a 1:1
association between a vCPU id and the offset (VP id) of a VP
structure within a fixed size block of VPs. We currently try to
enforce the 1:1 relationship by checking that a vCPU with the
same id isn't already connected. This is good but unfortunately
not enough because we don't map VP ids to raw vCPU ids but to
packed vCPU ids, and the packing function kvmppc_pack_vcpu_id()
isn't bijective by design. We got away with it because QEMU passes
vCPU ids that fit well in the packing pattern. But nothing prevents
userspace to come up with a forged vCPU id resulting in a packed id
collision which causes the KVM device to associate two vCPUs to the
same VP. This greatly confuses the irq layer and ultimately crashes
the kernel, as shown below.

Example: a guest with 1 guest thread per core, a core stride of
8 and 300 vCPUs has vCPU ids 0,8,16...2392. If QEMU is patched to
inject at some point an invalid vCPU id 348, which is the packed
version of itself and 2392, we get:

Kernel panic - not syncing: Fatal exception

This affects both XIVE and XICS-on-XIVE devices since the beginning.

Check the VP id instead of the vCPU id when a new vCPU is connected.
The allocation of the XIVE CPU structure in kvmppc_xive_connect_vcpu()
is moved after the check to avoid the need for rollback.

The NULL pointer being dereferenced is mode.

Git bisect has identified the following commit as bad:
f28e32d drm/nouveau/kms: Don't change EDID when it hasn't actually changed

Here is the chain of events that causes the oops.
On entry to nouveau_connector_detect_lvds, edid is set to NULL.  The call
to nouveau_connector_detect sets nv_connector->edid to valid memory,
with status set to connector_status_connected and the flow of execution
branching to the out label.

The subsequent call to nouveau_connector_set_edid erronously clears
nv_connector->edid, via the local edid pointer which remains set to NULL.

Fix this by setting edid to the value of the just acquired
nv_connector->edid and executing the body of nouveau_connector_set_edid
only if nv_connector->edid and edid point to different memory addresses
thus preventing nv_connector->edid from being turned into a dangling
pointer.

If GPU components have failed to bind, shutdown callback would fail with
