It is possible that there is a bug in cornerstone. One way to check for this is to load the same image in another DICOM viewer like Osirix. If the image displays fine in Osirix but not in cornerstone, there may be a bug. In this case you should de-identify the file and make it available for a cornerstone developer to debug.
