[Meta] create a pull request template

As mentioned in #1472 (comment), https://github.com/isaacs/github/community highlights that we don't have a pull request template for this repository. This seems like a good thing to add, so I'm filing this issue to track the task of adding one (or at least deciding that we don't need one and documenting that decision).
